OverviewShenandoah Oncology is looking for a detail-oriented individual with a compassionate and professional attitude to join our Quality & VBC Team!Our Quality Data Coordinator is responsible for the collection, coordination, processing and quality control of Value Based Care (VBC) data, and other Quality & VBC files as applicable. The Quality Data Coordinator works with Quality & VBC team and clinical staff to meet VBC data requirements and communicate any concerns about patients and/or data.ResponsibilitiesEssential duties and responsibilties include (but not limited to):* Extrapolate data from source records to identify and maintain reports of high-risk patients, pain scores, hospital discharges, and patient deaths in accordance with VBC requirements. Notify practice staff of hospital discharges and deaths as needed.* Perform follow-up calls to VBC patients and communicate with Skilled Nursing Facilities (SNF) and/or Home Health after hospital discharge.* Contact patients who meet high-risk criteria to monitor status until it is determined the patient is safe, stable, or treatment complete.* Contact patients with qualifying pain scores to review or complete a pain plan of care. Perform follow-up calls to ensure pain regimen was effective, discuss understanding, and offer minimal clinical advice regarding patient's pain.* Communicates Quality & VBC feedback and patient queries from follow-up or monitoring calls to physicians and/or clinical staff.* Assist the Quality & VBC team with ensuring accurate and detailed communication between physician and patients utilizing MyCarePlus, a patient portal.* Actively participates in VBC meetings and reviews.* Ensure accurate and adequate documentation to meet VBC reporting guidelines and goals.* Ensure effective use of EHR technology for all patients to satisfy Promoting Interoperability program components and meet CMS reporting requirements.* Adheres to confidentiality, state, federal, and HIPPA laws and guidelines with regard to patient records.* Assists and supports the implementation of practice wide clinical quality initiatives.* Performs other duties as requested or assigned.QualificationsHigh school diploma or equivalent required. Associates Degree strongly desired; Bachelor's Degree preferred. 0-3 years work experience required. Some medical office experience required; oncology preferred. Value Based Care (VBC) experience strongly desired. Experience with computer data entry and database management required.
